                                  Work Permits

Students ages 14-17 seeking employment will need to have a parent/guardian complete a work permit application.  

The parent/guardian may come in to complete the application, they must also provide evidence of the student's date of birth.  If they are unable to come to the school, the student may take an application home for the parent/guardian to complete and return along with proof of birth.  

 

A work permit (blue card) is issued to the student when:

 

Application signed by parent/guardian is returned to the Guidance Secretary, with evidence of age

Permit Number is issued and recorded on the application and the blue permit card

Blue permit card must be signed by student in the presence of the school issuing officer (Guidance Secretary)

                                                SAT Testing Photo ID

It is mandatory for students taking the SAT test to have a photo ID along with their photo registration ticket. The photo must be acceptable. See your school counselor with any questions.

 

Students needing a photo ID can see the Guidance Secretary to obtain one. 

                2019-2020 Denial of Access to Military Recruiters Opt-Out Form

The Military Opt-Out Form will be printed and put in the Junior/Senior homeroom mailboxes. The first day of school students will receive these along with other important correspondence and take home August 23, 2018

 

Parents are permitted 21 days to respond to this request by completing the form and returning it or by contacting the Guidance Secretary at 724-842-0416. After this time, September 14, 2018, recruiters will be given a list of the current Juniors/Seniors upon request. The list contains names, addresses and phone numbers.

Transcript Request
 

Currently enrolled students or past graduates of Kiski Area High School may be issued an unofficial copy or an official sealed copy of their transcript upon request. 

 

Transcript release forms are available below or in the high school guidance office.

 

Once you have completed and signed the release form you may email it to 

 lisa.williams@kiskiarea.com,

fax it to 724-842-0403, or drop it off at the High School.
